Develops and implements individualized plans of care based on patient assessment and needs.
Collaborates with healthcare team members to ensure seamless care coordination and communication.
Evaluates patient response to interventions and revises plan of care as needed.
Identifies opportunities for quality improvement and implements recommendations.
Provides education and support to patients and families to promote health and well-being.Requirements
- Associate's Degree in Nursing.
- One year experience as a registered nurse.
