Job SummaryWe are seeking a compassionate and skilled Nurse Care Coordinator to join our team at Prisma Health. As a key member of our healthcare team, you will provide clinical direction and coordination of patient care in an acute inpatient setting, fostering patient-centered care and promoting a seamless transition from one level of care to another.Responsibilities

• Collects data and completes nursing assessments to identify patient priorities and develop individualized plans of care
• Provides direct care to assigned patients, prioritizing their needs and coordinating with other healthcare team members to ensure effective care delivery
• Delegates nursing tasks to members of the healthcare team, ensuring completion of assigned tasks and maintaining a safe environment
• Monitors quality of care delivered and identifies opportunities for quality improvement, incorporating performance improvement recommendations into the plan of care
• Evaluates patient response to interventions, revises the plan of care as needed, and reports changes in patient condition to the healthcare teamQualifications
  • Associate degree in Nursing; Bachelor's degree in Nursing preferred
  • One (1) year experience as a registered nurse; successful completion of the Prisma Health Nurse Residency Program or a comparable residency from another organization may be considered in lieu of experience
  • Licensed to practice as a Registered Nurse in South CarolinaEstimated Salary Range$65,000 - $85,000 per year, depending on experienceThis is a full-time, night shift position at Greenville Memorial Hospital.
