Transforming Healthcare TogetherJob SummaryEvaluate and
coordinate patient readiness and transfer for post-acute services
within Prisma Health, ensuring quality and cost-effective clinical
resource utilization. Key responsibilities include patient and
family education, discharge planning, and financial assessment for
Subacute, LTC, Inpatient Rehabilitation, and Long Term Acute Care
services. AccountabilitiesPatient Assessment
- Assess referred patients to identify the need and
appropriateness for Post-Acute Services.
- Communicate results of physical, psychosocial, and financial
assessments with patients, physicians, and healthcare teams.
- Collaborate with Discharge Planners to formulate
patient-centered plans.
- Maintain in-depth understanding of admission criteria for
post-acute care services.Planning
- Work with patients, families, and healthcare teams to formulate
placement plans, updating goals and plans daily as needed.
- Attend discharge rounds to facilitate complex patient
management and recommend post-acute care services.
- Maintain open communication with healthcare teams regarding
patient status and potential next steps.Implementation
- Involve patients and families in admission planning and
preparation for discharge, considering cultural, ethnic, and
religious beliefs.
- Advise discharge teams on patient readiness for community
discharge.
- Conduct preadmission physical assessments and rehab evaluations
to determine placement.
- Identify and address potential delays in care provision.
- Integrate knowledge of payor requirements and federal/state
regulations for post-acute care services.
- Collaborate with family and financial counselors to optimize
resource utilization.
- Provide patient and family education for continuum of care and
community transition.
- Implement guidelines established by regulatory and accrediting
agencies.Program Tracking and Evaluation
- Monitor and analyze clinical and fiscal outcomes, presenting
data to management.
- Collect clinical data for evaluation of CMG's and RUGS,
ensuring compliance with government regulations.
- Participate in program evaluation, CQI studies, and performance
improvements for post-acute facilities.
- Maintain database of referrals and admissions for reporting
purposes.
- Collaborate with team members to perform quarterly program
quality audits, identifying opportunities for
improvement.Leadership
- Act as a clinical resource for staff, maintaining strong
knowledge base in post-acute care services.
- Provide information and resources on specialty rehab services
through the GHS website and toll-free telephone number.
- Participate in management groups, nursing committees, and
quality improvement projects, demonstrating leadership.
- Serve as a clinical resource to the interdisciplinary
healthcare team.
- Orient and train new employees on post-acute admission criteria
and department policies.
- Seek opportunities for continuing education and professional
development.Program Marketing and Network Development
- Market post-acute services to hospitals and physicians within
service areas.
- Create and deliver presentations on post-acute services to
healthcare professionals.
- Develop and maintain strong referral sources within the
Greenville Health System.Supervisory/Management

EducationAssociate's Degree - NursingMinimum Experience2 years of
post-acute related experienceRequired
