Role Overview: We are seeking a skilled and compassionate Registered Nurse to join our team at Trinity Home Health. As a Registered Nurse, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.About the Position: The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Your key responsibilities will include:

• Coordinating Care: You will work closely with other healthcare professionals to ensure seamless care coordination and achieve continuity of patient care.
• Developing Care Plans: You will conduct initial and comprehensive nursing evaluations, develop care plans within state-specific guidelines, and submit accurate and timely documentation.
• Providing Patient-Centered Care: You will ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.Requirements: To be considered for this role, you must have:
  • Current RN Licensure: In the state of practice
  • CPR Certification: Current and valid
