Call Rotation: Some nights and weekends phone call ( if included in
contract)Location: Provider will travel locally to see patients in
Palliative Care in the following counties - Buncombe, Haywood, and
Madison. Mileage Reimbursement Included.Position Summary:Provide
patient care consultation and treatment to patients with serious
illness with support from a collaborative physician.Essential
Duties & Responsibilities:Work to achieve performance goals, pursue
progress toward individual development plan (IDP) and demonstrate
Four Seasons values and competenciesProvide direct client care
consultations, to include participation in patient and family
counseling and management of symptoms related to life-limiting
conditions in collaboration with the interdisciplinary team of
physicians, nurses, social workers, and chaplainsTake clinical call
for palliative care clientsActively participate in
Interdisciplinary Team meetings for development and evaluation of
the clients plan of careCreate and maintain accurate patient
recordsPractice within the scope of license, certification or
registration, where applicable, and as required by law and
regulationServe as clinical resource for staff of Four Seasons,
staff at facilities served, and providers in the communityMaintain
current medical education and appropriate licensurePerform other
duties as requested or assignedKnowledge, Skills &
Abilities:Knowledge of Federal, State and Local regulations and
Medicare Conditions of ParticipationKnowledge of HIPAA privacy
rules and regulationsKnowledge of electronic medical records
systemsBasic data entry skillsExcellent organization

PA Board Approved CertificationCurrent NC Medical License in good
standingCurrent DEA Registration in good standingMeet and maintain
eligibility requirements to be appointed medical staff and qualify
for privileges at partner facilitiesValid driver's license and
proof of automobile insurance.Summary of Physical
