Job SummaryAs a Wound Care Specialist at Prisma Health, you will play a vital role in providing care and consultation for patients with complex wound care needs. Your expertise will serve as a resource for healthcare professionals, patients, and families, helping to design systems that facilitate the appropriate care of this patient population.Key Responsibilities

• Assess the physical and educational needs of patients undergoing ostomy surgery or patients with wounds, fistulas, incontinence, percutaneous tubes, or pressure ulcers.
• Communicate patient needs to the healthcare team and provide discharge teaching as requested.
• Identify opportunities for system-wide improvement in skin, ostomy, wound, or continence care, developing and revising Nursing Policies and Procedures, Standards of Care, and protocols based on current research.
• Evaluate the outcomes and effectiveness of system-wide improvements.
• Develop plans for care management based on patient needs through consultation and collaboration with other healthcare team members, coordinating care in an efficient and cost-effective manner.
• Implement and evaluate the plan of care for individual patients, based on protocols, procedures, and guidelines.
• Identify actual/potential issues of resource management/patient care, working with the healthcare team to identify cost-effective alternatives, and facilitate discharge.
• Educate patients, families, and staff on evidence-based skin and wound care practices, promoting healthcare in the community through education.
• Promote role identification by serving as a clinical preceptor to undergraduate nursing students and staff.
• Coordinate meeting agendas and issue follow-up for the Prisma Health Skin Care Committee, evaluating products used, including specialty beds and mattresses, and presenting findings and making recommendations to the committee.
• Coordinate quarterly Pressure Ulcer Prevalence Studies and facilitate performance improvement activities related to the analysis of results, serving as a resource.
